Overview

This webinar series will provide strategic, business and clinical leaders within hospitals and health systems the skills, information and implementation tools they need to move forward with Population Health Management. Attendees will learn what has worked at other hospitals and organizations and how to address challenges that are unique to California.

This is an opportunity to lead change at your organization. Registered participants who complete all five webinars will earn a certificate in Population Health Management (PHM).  Each webinar features an issue brief appropriate for executives and professionals in a wide range of organizations, whether already immersed in PHM or just formulating their PHM strategy. CHA has partnered with management consultants and thought leaders from Kaufman Hall to develop and present this program.

Webinar 1

 A Framework for Population Health Management — April 28, 2015

We begin by defining PHM and describing its rationale and purpose. Participants will learn the key components of PHM, the competencies needed for success, and the different roles that organizations play within a PHM framework. Also included will be an overview of the PHM activity in California markets, along with a roadmap that organizations can use to develop or strengthen their PHM strategy.

After viewing a recording of the module, participants will be able to:

  • Define key competencies related to PHM, including network strength, clinical integration, operational efficiency, and clinical and business intelligence;
  • Assess organizational strength in each competency and understand how to fill any competency gaps;
  • Describe how California’s health insurance and delivery markets currently reflect PHM characteristics and will continue to evolve;
  • Define roles for provider organizations in future PHM, including population health manager or co-manager, multiproduct participant, single product participant, and contracted participant; and
  • Use a seven-step roadmap for understanding and organizing around PHM, including phases for evaluating the market context, identifying a PHM value proposition, and developing a PHM plan.

Webinar 2

Business Imperatives of Population Health Management — June 2, 2015 (recording available)
This module will address the business challenges in developing PHM care models and delivery networks, including how to configure a network to achieve adequate coverage, desired market position, and high-quality care and positive outcomes. Build, buy or partner options for network development will be explored, along with key business skills in operating a successful PHM delivery network, including contracting, cost management, consumer engagement, and marketing and communications.

After viewing a recording of the module, participants will be able to:

  • Describe approaches to population attribution, integration and funds flow related to physician and clinical integration;
  • Understand the role of delivery network optimization and contracting in population health success;
  • Define marginal cost reduction versus total cost transformation and their role in population health management; and
  • Describe elements of the management/governance, care management, IT and analytics infrastructure necessary for the business facets of population health management.

Clinical Imperatives of Population Health Management — July 14, 2015 (recording available)
Use of consistent clinical protocols within well-defined care models is critical to successfully managing the health of a population under value-based payment or an at-risk financial model. Learn about wellness models, chronic disease management protocols, and evidence-based practices for effective health maintenance, with an emphasis on evolving roles among clinicians. Tools to analyze population health needs and coordinate clinical care in all settings and modalities, including virtually, will also be covered.

After viewing a recording of the module, participants will be able to:

  • Describe the importance of segmenting the population according to health status, and epidemiology;
  • Identify different types of clinical interventions that apply to different population segments; and
  • Define the role of intervention evaluation and refinement in achieving desired benefits and outcomes.

Technology for Population Health Management — August 25, 2015
The technology needs of PHM are significantly different than those of traditional models. This module will cover the key technology capabilities for PHM, including care coordination monitoring and tracking; patient-centric disease registries; advanced health analytics, including predictive modeling; enterprise risk management; cost management; and virtual/mobile health. In addition, participants will learn the importance of integrating clinical and administrative information, new areas of required technology expertise, and how to assess capital requirements for technology investments.

After this webinar, participants will be able to:

  • Describe the role of healthcare IT in the transformation to high-value care;
  • Describe the relationship between organizational strategy and healthcare IT needs;
  • Define four roles of healthcare IT in the care management platform; and
  • Describe the vendor marketplace for healthcare IT related to PHM.

Webinar 5 

Leadership and Talent for Population Health Management — October 6, 2015
PHM makes new demands on leadership and talent. This session will define the leadership and governance mindset for PHM, including new perspectives on scale, volume and partnerships. The module will describe the role of leadership in fostering key PHM capabilities and introduce new leadership positions that focus on important facets of the PHM model, such as chief transformation officer, chief population health officer, chief medical informatics officer, and a leader for product and benefit design. Finally, participants will learn new metrics of leadership performance and accountability for a PHM environment.

After this webinar, participants will be able to:

  • Describe new leadership perspectives on scale, volume and partnerships for population health management
  • Identify new roles and competencies for management and governance, including network development, risk contracting and innovation
  • Define new governance and leadership structures for population health management

Faculty Bios

Kenneth Kaufman, Chair, since 1976, Mr. Kaufman has provided health care organizations with expert counsel and guidance in areas including strategy, finance, financial and capital planning, and mergers, acquisitions and partnerships. Clients include organizations of all types and sizes – community hospitals and health systems, academic medical centers, and regional or national health systems.

Recognized as a leading authority, Mr. Kaufman is a frequent speaker at meetings of the American College of Healthcare Executives (ACHE), American Hospital Association, Healthcare Financial Management Association and The Governance Institute (TGI). Mr. Kaufman has authored or coauthored six books, most recently authoring Focus on Finance, published by TGI, and Best Practice Financial Management, published by ACHE.

Mark Grube, Managing Director, leads Kaufman Hall’s Strategic Advisory practice. Mr. Grube has more than 25 years of experience in the health care industry, as a consultant and as a planning executive with one of the nation’s largest health care systems. Mr. Grube is a frequent speaker and author and he is a three-time winner of the Helen Yerger/L. Vann Seawell Best Article Award from the Healthcare Financial Management Association. He has presented at meetings of the American College of Healthcare Executives, The Governance Institute, The Healthcare Roundtable, HFMA, and the Society for Healthcare Strategy and Market Development.

Pam Arlotto, President and CEO, has blazed new trails in health care for more than 30 years. Under her leadership, Maestro Strategies was one of the first consulting firms to align health care business and clinical strategy with information technology strategy. Pam’s expertise includes: board and c-suite education; creating strategies for clinical integration; reinventing healthcare business and delivery models; value propositions for HIT service and product companies; the c-suite’s role in clinical transformation; and the integration of quality, performance improvement and information technology.

Pam is well known for the award winning book series, Beyond Return on Investment: Expanding the Value of Healthcare Information Technology. Pam has been featured on National Public Radio and in the Wall Street Journal.

Todd Fitz’s responsibilities include strategic planning assistance to hospitals and health systems nationwide, focusing on network development, physician integration and governance, performance improvement, and academic medical centers (AMCs) strategy. With more than 18 years of health care experience, Mr. Fitz has applied his expertise in finance, strategy and operations to a variety of settings, including community hospitals, physician groups, AMCs, insurance organizations, government agencies and medical device companies both in the U.S. and internationally. He is a frequent contributor of thought leadership publications and presentations.

Jody Hill-Mischel has more than 30 years in health care consulting, her expertise includes strategic financial and capital planning, strategic options assessment, merger, acquisition, divestiture, and partnership arrangements, and capital and financial advisory services. Ms. Hill-Mischel’s clients include health care systems, academic medical centers, community medical centers, and physician groups. Ms. Hill-Mischel has written for healthcare professional journals, including hfm magazine, and is a regular speaker on health care strategy and finance topics.

Anand Krishnaswamy is an assistant vice president in Kaufman Hall’s Strategy practice. His responsibilities focus on providing analytical support in the execution of transactions and technical/resource support for practice operation. Prior to joining Kaufman Hall, Mr. Krishnaswamy worked as a senior financial analyst at Lehman Brothers, where he performed mark-to-market valuations for the firm’s subprime mortgage lending business and for mergers and acquisitions in the consumer asset space. Mr. Krishnaswamy also was a senior associate consultant at Mars & Co., where he modeled and analyzed cost optimization structures in the Northeast and Texas regions for a Fortune 500 consumer products company.

Matthew J. Lambert III, MD, has more than 40 years of health care experience working as a physician, health care executive and board member. His current work at Kaufman Hall (KH) is focused on assisting hospitals and health systems with integrated strategic and financial planning, service line planning/distribution across systems, and medical staff planning/physician alignment strategy.

Prior to joining KH, Dr. Lambert held senior executive positions at several Chicago area hospitals. He also has provided consultation to hospitals and health systems in the areas of physician relations and the continuity of care. Dr. Lambert served on the board of directors for a multihospital system, including as vice chair. He also has served on both the American Hospital Association’s Leadership Development Council and Committee on Governance. Dr. Lambert was a regent for the American College of Healthcare Executives and developed and leads its annual Physician Executive Boot Camp for physicians new to management. He also has served on the Baldrige Performance Excellence Program’s Board of Examiners, and as an examiner for the Washington State Quality Award program.

Walter Morrissey, M.D., has more than 18 years of health care experience working as a clinician, strategic advisor and administrator. He is a member of Kaufman Hall’s Strategy practice, and is focused on assisting hospitals and health systems with designing and rationalizing regional delivery networks, clinical and physician alignment strategy, and integrated strategic and financial planning to prepare for the transition to value-based healthcare.

Prior to joining Kaufman Hall, Dr. Morrissey was a vice president at Sg2, an international health care intelligence and consulting firm. He has worked with hospitals, health systems, device manufacturers, and venture firms both in the U.S. and around the world.

John Poziemski focuses specifically on integrated strategic and financial planning, strategic options evaluation, mergers and acquisitions, and value-based transformation initiatives, working primarily with provider organizations in the western region. Prior to joining Kaufman Hall, Mr. Poziemski worked at Sg2, where he managed a variety of consulting engagements and business-development activities focused on clinical program development and physician integration strategy.

Kalani Redmayne is an assistant vice president at Kaufman Hall and brings over 20 years of health care industry experience to her work as a member of the firm’s Strategy practice. In this role, she provides strategic planning advisory services for a wide range of clients, including health care systems, academic medical centers, and community hospitals. Ms. Redmayne’s responsibilities focus on payer-provider strategies, alternative reimbursement methodologies, health plan and service operations, health care reform, payer relations, and product development. Prior to joining Kaufman Hall, Ms. Redmayne was vice president of Product Development and Management for UnitedHealthcare, Inc., where she focused on Medicare Advantage, Medicare Supplement, Prescription Drug Plans and UnitedHealthcare’s private exchange products.

Rob York is a senior vice president of Kaufman Hall and leader of the Population Health Management division in the firm’s Strategy practice. He provides strategic services for a range of healthcare clients, including large health care systems, public/safety-net providers, academic medical centers, and community hospitals. Mr. York’s responsibilities focus on developing strategies to help providers remain relevant and viable in the new healthcare environment based on rigorous market analysis, population and payer segment and demand analysis, and strategic partnership evaluation.

He regularly speaks to Boards and at meetings of professional societies, and has published numerous articles in industry journals, including Health Affairs, Spectrum (a publication of the American Hospital Association’s Society for Healthcare Strategy and Market Development), and Strategic Financial Planning (a newsletter from the Healthcare Financial Management Association). Prior to joining Kaufman Hall, Mr. York was a consultant with KPMG LLP’s West Coast and Midwest Healthcare Practices.

Cancellation Policy/Late Payment: A $50 non-refundable processing fee will be retained for each cancellation. PHM Certificate

CHA will award a Certificate of Population Health Management to each registered individual that participates in all five webinars. In order to receive the certificate, participants must:

  • Register for the complete five webinar series
  • Fully participate in each webinar and attest to participation
  • Complete a post-event survey

If you are not available on the day of a scheduled webinar and therefore cannot participate, you may request access to the webinar recording, to be viewed at a later date. Requests to access the recording will be limited to two webinars in the series.

Important note: While more than one individual from your organization may listen to the webinars, CEs and the Certificate of Population Health Management will only be awarded to the registered participant.
