Social and Community Context

Recognizing the connection between people’s health and well-being and the contexts within which they live, learn, work and play, California’s hospitals have developed innovative programs and activities that promote community cohesion, support systems and civic participation — and others that address abuse, trauma and/or violence, grieving, human trafficking and incarceration.


Building Stronger Families Project: Improving access to social services in the Los Angeles area.

Intergenerational Elective Program: Connecting middle school students and nursing home residents in San Francisco.

Latino Health Promoters Program: Community health workers assisting peers in Southern California.

Monument Community Senior Services Outreach Program: Promoting safety and well-being among seniors in the Monument Corridor.

Promotores/Community Health Worker Pilot Certification Program: A hospital-funded grant to Planned Parenthood Northern California piloting a community health worker certification program.

Promotores de Salud/Health Promoters Program: Bridging language and culture to provide health education and access through trusted community health promoters in the Santa Rosa area.


A Caring Community of Education, Services & Support (ACCESS): A biweekly car seat fitting station for babies and children.

Caring Hands Volunteer Caregivers Program: Helping seniors in Northern California remain independent by matching them with caring volunteers.

Community Health Wellness and Activity Center: Partnering with local volunteers and community organizations near Wilmington to offer free classes and activities that promote wellness.

Hope Street Margolis Family Center: Offering services that enhance parents’ and families’ capacity to nurture and care for their children in Los Angeles.

House Calls Program: Providing primary care for homebound, frail elders and adults with chronic diseases in the Santa Rosa area.

Kol Haneshama: Jewish End of Life Care Volunteer Program: Volunteers partnering with hospice patients in need of spiritual support in the Bay Area.

Life Learning Center: Providing classes in the Santa Rosa area that encourage community members’ personal and professional development.

Live Well Senior Program: Helping individuals 55+ in Glendale, and their caregivers, to navigate the health care system.

Mindful Life Project: Using new practices for healing that focus on mindfulness in the Bay Area.

Mental Health Awareness Educational Outreach Program: A student-led club that helps raise mental health awareness in the San Gabriel Valley.

PossAbilities Community Outreach Program: Offering disabled individuals a sense of community and a healthy social network in the Loma Linda area.

Therapeutic Preschool (Day Treatment Intensive Program): Providing access to intensive services for preschool-aged children with behavioral and learning challenges.

Volunteer Doula Program: Helping mothers with childbirth through physical, emotional and informational support in the Davis area.


Live Well Senior Program: Serving people 55+ in the Glendale area with access to health care information, resources and wellness/prevention programs.

Monument Impact: Training and tools for economic self-sufficiency, civic engagement, connection to others and lifelong learning.


Beyond Violence: A community collaboration in Contra Costa County, transitioning trauma patients from the hospital to their communities.

Center for Child Protection: Offering medical and mental health services to children affected by abuse and exposure to violence in Alameda County.

Sexual Assault Response Team Center (SART): Collaboration between the hospital, law enforcement, forensic nurse specialists and counselors to offer compassionate treatment to sexual assault victims in San Pedro and Torrance.

Share and Care: Helping to heal psychological wounds in children in the Los Angeles area who have witnessed or experienced trauma.

Teacher Training Program: Providing teachers in the Los Angeles area with tools and techniques to help struggling students who have experienced trauma.


Bereavement Education & Support Group, Spousal Loss Grief Support Group: Helping individuals cope with loss or illness through support groups in the Redding area.

Children’s Bereavement Art Group: Assisting grieving children in the Sacramento area by encouraging creative expression, and with peer support.


Human Trafficking Response Program: Ensuring trafficked individuals are identified in the health care system and given trauma-informed care.


Liberty Tattoo Removal Program: Improving employment and educational opportunities by removing anti-social and gang-related tattoos for individuals in San Luis Obispo.  

Tattoo Removal Program: Providing gang intervention specialists and tattoo removal services in the northeast San Fernando Valley.
