San Francisco Chronicle

State health officials issued two citations each to California Pacific Medical Center in San Francisco and UCSF Medical Center for safety violations that were serious enough to potentially cause patient death or injury, three of which involved objects left inside patients after surgery.

The San Francisco hospitals were among 12 California hospitals hit with administrative penalties ranging from $25,000 to $75,000, the state Department of Public Health reported Friday.
