Medical costs driven by unhealthy lifestyles, says insurance executive

U.S. health insurance premiums rose more than 134 percent since 2002, driven largely by out-of-control costs of medical treatment, waste and fraud in health care, and people not taking care of themselves, the president of Anthem Blue Cross California said Thursday.

Speaking to a crowd of 300 brokers and others in the insurance industry at a Woodland Hills hotel, Pam Kehaly tossed out statistics to punctuate the point.
