Laguna Honda asked to check error files

Laguna Honda Hospital was one of 87 hospitals asked by state public health officials to look more carefully at their files because they have not reported a single medical error since 2007.

Over that time, all acute-care hospitals have been required to report a host of medical errors to state officials, but about 20 percent of the state’s hospitals have failed to report even one, according to an investigation by the Los Angeles Times.
