Errors may have put 3 hospitals on state list — for no errors

Three local hospitals made the list of 87 hospitals in California that haven’t reported any “adverse events” to the state since 2007. However, local hospital officials say they have reported medical errors, as required by the state. So they may be on the mistake-free list by, well, mistake. An “adverse event” includes 28 categories that hospitals are required to report. They range from surgery at the wrong site to medication errors to the death of a baby.
