Dominican Hospital was fined $50,000 Thursday by the state Department of Public Health for a fatal lapse in monitoring a patient on a ventilator.

State regulators said Dominican was among a dozen hospitals fined a total of $650,000 for mistakes made between 2008 and 2010 that endangered patients.

Other errors triggering fines involved a cancer patient who died after getting an overdose of morphine at a hospital in Escondido, a patient who died after he fell and fractured his skull while hospitalized in Poway, and a patient at a San Diego hospital who died from an overdose of a medication to treat congestive heart failure.
