Children’s Hospital Oakland fined

State investigators fined Children’s Hospital Oakland on Tuesday for failing to provide adequate controls and policies in the wake of violent and potentially unsafe situations at the hospital.

The state’s Division of Occupational Safety and Health, known as Cal/OSHA, issued the safety violations in connection with two incidents, one of which happened in July when a homeless man armed with a gun burst into the emergency room and briefly took an employee hostage before surrendering.
