A breakdown in hospital protocol led to the death of a Sutter Davis Hospital patient two years ago, according to the state Department of Public Health, which Tuesday fined the hospital $25,000 for the lapse.

The woman, who was not identified by state officials, died in March 2008 after succumbing to an allergic reaction while undergoing treatment in the hospital’s emergency department, according to the state agency.
