Under Senate Bill 1254 (Chapter 697, Statutes of 2018), hospitals must establish criteria for high-risk patients and complete a medication profile for those patients. To better understand how this new requirement will impact patient safety, CHA and California Health System Pharmacists members will undertake a six-week, multi-center quality improvement project.
During the project, which will take place in early 2020, participants will be asked to document and analyze discrepancies and harms avoided, as well as economic impact. To participate, sites must have an active pharmacy medication history program, the ability to track and document medication history errors, a site coordinator to oversee the project, and a physician to review errors. Participants will be required to join twice-monthly calls beginning in August 2019.
