State to Submit Final Health Care Innovation Plan

The California Department of Health and Human Services (HHS) will submit its final State Health Care Innovation Plan to the Center for Medicare and Medicaid Innovation (CMMI) at the end of this month. The attached infographic summarizes the innovation plan’s vision, goals, four initiatives and six building blocks. HHS plans to move forward on several fronts to maintain planning momentum until testing funds are available from CMMI. Activities will include developing cost and quality metrics and payment reform options for maternity care; thinking through a health home for complex patients program, including integration of palliative care; developing goals and outcomes for accountable care community (ACC) pilots, including a local wellness trust; and analyzing ways to foster incorporation of community health workers as team members within health homes for complex patients, palliative care services and an ACC.
