Revised POLST Forms Available for 2016

Revised Physician Orders for Life Sustaining Treatment (POLST) forms will go into effect Jan. 1, 2016, as the result of new legislation that allows nurse practitioners (NPs) and physician assistants (PAs) operating within their scope of practice and under the supervision of a physician to sign POLST forms. The optional POLST form, which may be completed with patients with a serious illness or whose life expectancy is one year or less, documents the patient’s wishes concerning medical treatment and interventions at life’s end.

The new form, attached, may be used beginning Jan. 1, 2016. NPs and PAs are advised to use only the revised POLST form because previous versions do not contain necessary signature lines or clarifying language. Previous versions signed by a physician remain valid.
