Provider Organization Roles Topic of New CHA Podcast

This week’s installment of CHA’s Population Health Management (PHM) Snapshot podcast series focuses on the different provider roles that have emerged under a PHM construct. Ranging from population health managers and co-managers to single product, multiproduct and contracted participants, each role encompasses varying levels of provider risk. Also taken into account are the organization’s strategic-financial condition, its competencies,  community readiness for PHM, and the current and emerging PHM environment.
