Post-Acute Care Conference Will Focus on Quality Outcomes, Increased Care Coordination

The annual Post-Acute Care Conference will be held Feb. 11–12, 2016, at the Crowne Plaza Redondo Beach. This year’s program, Break the Mold — The New Shape of Post-Acute Care, will address how health care is evolving for all providers and the importance of integrating services with post-acute care. Agenda highlights include the application of population health management to post-acute care delivery; ways to use community-based organizations to improve transitions of care; updates on the Improving Medicare Post-Acute Care Transformation (IMPACT) Act implementation and the Comprehensive Care for Joint Replacement model; the impact of new payment models on post-acute care delivery; and an update from the Centers for Medicare & Medicaid Services, Region IX and the Center for Medicare and Medicaid Innovation. Interactive roundtable discussions followed by a panel presentation will highlight innovative practices for improving outcomes of care.
