Post-Acute Care Conference Emphasized Collaboration, Community

Over 130 health care professionals attended last week’s Post-Acute Care Conference, which emphasized strengthening connections across the continuum of care. The conference encouraged attendees to collaborate to support their patients’ unique needs and to build the strong relationships that are crucial to minimizing risk and achieving cost and quality goals. Following the theme of creating a community of care, CHA’s Vice President, Continuum of Care Pat Blaisdell spoke on the importance of making connections with patients, families and caregivers. Other presentations addressed the Improving Medicare Post-Acute Cart Transformation (IMPACT) Act and upcoming implementation of a new workplace violence prevention regulation.
