New Hand-Off Communications Tool Released

The Joint Commission Center for Transforming Healthcare has released a new Targeted Solutions Tool (TST) for Hand-Off Communications to assist organizations with the process of passing necessary and critical information about patients from one caregiver to the next to prevent miscommunication-related errors. An estimated 80 percent of serious medical errors involve miscommunication among caregivers when patients are transferred or “handed off.” In addition to patient harm, defective hand-offs can lead to delays in treatment, inappropriate treatment and increased lengths of stay in the hospital. The Joint Commission reports that health care organizations using the tool have reduced readmissions by 50 percent, and reduced the time it takes to move patients from emergency departments to inpatient units by 33 percent. Health care organizations also report an increase in patient and family satisfaction, staff satisfaction and successful transfers of patients. For more information, go to
