Final Series Installment Helps Organizations Identify Appropriate PHM Path


The last installment of CHA’s Population Health Management Snapshots series details the final level of the seven-part framework to pursuing PHM opportunities — identifying the appropriate PHM path for your organization. To achieve PHM expertise, hospitals and health systems should explore all available options, including building internal competencies and capabilities, buying or purchasing access to certain competencies from another entity, or partnering to gain access to required competencies. To participate in PHM in a meaningful way, most organizations will need to use the latter two approaches.
