DHCS Provides Update on Medi-Cal Health Homes Program

The California Department of Health Care Services (DHCS) has announced it is currently working with San Francisco Health Plan, Anthem Blue Cross and Partnership Health Plan of California to implement the first wave of the Health Homes Program (HHP), to be implemented in 11 counties as identified in DHCS’ revised county implementation schedule. Two subsequent waves with additional counties will follow. DHCS announced it is awaiting Centers for Medicare & Medicaid Services approval of the state plan amendment that it submitted requesting authority to begin implementing the first wave of the county-based, managed care plan-delivered HHP in July 2018. 

The HHP will serve eligible Medi-Cal beneficiaries with multiple chronic conditions who are frequent utilizers of health care services and who may benefit from enhanced care management and coordination. The program provides six core services:  comprehensive care management, care coordination (physical health, behavioral health and community-based long-term services and supports), health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services. For more information about the HHP, visit the DHCS website.
