The Department of Health Care Services (DHCS) has released an update on the Health Homes Program, including current and future implementation counties, and counties where the program will not be implemented.
On Jan. 1, program implementation began in Riverside and San Bernardino counties for eligible members, based on chronic physical conditions and substance use disorders criteria. Health Homes Program services were also expanded in San Francisco County for members eligible because of serious mental illness or serious emotional disturbance.
In addition, DHCS announced that Partnership Health Plan of California and Central California Alliance for Health will not implement the program as previously anticipated, so the Health Homes Program will not be implemented in Del Norte, Humboldt, Lake, Lassen, Marin, Mendocino, Merced, Monterey, Napa, Santa Cruz, Shasta, Siskiyou, Solano, Sonoma, and Yolo counties.
DHCS is working with the remaining managed care plans in 11 additional counties (Alameda, Fresno, Imperial, Kern, Los Angeles, Orange, Sacramento, San Diego, San Mateo, Santa Clara, and Tulare) to prepare for their implementation in Group 3 beginning on July 1. Services will be provided in comprehensive care management, care coordination (physical health, behavioral health, community-based long-term services and supports), health promotion, comprehensive transitional care, individual and family support, and referral to community-based and social support services.
