CMS Issues Changes to Restraint and Seclusion Reporting Requirements

The California Department of Public Health has issued an All Facilities Letter (AFL) notifying hospitals of changes to the current federal reporting procedures for hospital deaths associated with the use of restraints or seclusion. Hospitals are no longer required to report deaths to the Centers for Medicare & Medicaid Services (CMS) if there was no use of seclusion and the only restraint was a two-point soft wrist restraint. Additionally, a new form — CMS-10455 — is now required when reporting a death, and no other forms will be accepted. See the attached AFL and other documents for additional reporting requirements and important information. AFL14-15 supersedes AFL 12-13.
