Passed by Congress in September 2014, the Act requires post-acute care providers— including long-term acute care hospitals, inpatient rehabilitation facilities, skilled-nursing facilities and home health agencies — to submit standardized patient assessment data, as well as quality and resource-use measures. The data are intended to facilitate data exchange among post-acute care providers and allow for longitudinal information to support care coordination and improved patient outcomes.
