CMS Announces 2 New California CCTP Participants

The Centers for Medicare & Medicaid Services (CMS) has announced 17 new participants in the Community Based Care Transitions Program (CCTP), including two from California. Mandated by the Affordable Care Act, CCTP provides funding to hospitals that partner with community-based organizations to improve transitions for high-risk Medicare beneficiaries. CCTP’s goals are to improve quality of care, reduce readmissions and document savings for the Medicare program. The Los Angeles Mid-City Integrated Care Collaborative and the San Francisco Transitional Care Program join the previously announced Advance Care Transitions in Marin County as California’s three participants in the program.
