CHPSO/ECRI Root Cause Analysis Web Seminar Set for Sept. 24

Learn about common issues associated with root cause analyses (RCAs) and how to submit an RCA for critique. The California Hospital Patient Safety Organization/ECRI Institute seminar, Explaining the Adverse Event, is open to members and nonmembers, Sept. 24 from 10 a.m. –11 a.m. (PT).

CHPSO has contracted with ECRI Institute to support confidential patient safety data collection and analysis. CHPSO assists California hospitals in making their institutions safer by examining what went wrong and why, and sharing lessons learned with other hospitals. Membership is complimentary for members of CHA and the Regional Hospital Associations.

As part of CHPSO membership, ECRI provides complimentary written critiques of a hospital’s RCA document, including assessment of thoroughness, identification of root causes and effectiveness of the action plan. This thorough evaluation is based on ECRI’s expertise in reviewing more than 2 million events to date. CHPSO member hospitals can submit up to six RCAs per year to ECRI through a secure CHPSO/ECRI portal.

To participate on Sept. 24, access the web seminar at For the audio, call (800) 619-0324, and enter 092412. No pre-registration is necessary.
