CHA Publishes Issue Brief on Population Health Management
Register today; program begins Tuesday

CHA has released an issue brief titled A Framework for Population Health Management that provides an overview of population health management (PHM) and a framework for its pursuit by hospitals and health systems. The comprehensive, 20-page document lays the foundation for the five-part PHM webinar series created by CHA for member hospitals, which will include a separate issue brief for each of the five webinars. Issue Brief 1 was emailed to all registrants today. The attached excerpt of the brief provides the five drivers of the PHM imperative, defines PHM and details nine key competency requirements.

“I encourage all member hospitals to participate in this program,” said Anne McLeod, CHA’s senior vice president, health policy and innovation. “PHM is the direction health care is moving, so simply stated, if participating in Medicare or Medi-Cal, all hospitals and health systems in California are in the PHM business. This program will help hospitals strengthen their position in the ever-changing health care delivery system.”

CHA contracted with management consultants and thought leaders Kaufman, Hall & Associates to write the issue briefs and create the curriculum for the certificate program. Program topics include a framework for population health, business imperatives, clinical imperatives, technology requirements, and new competencies in leadership and talent. Participants who complete all five webinars will earn a Certificate in PHM.

Individuals who are not able to participate on the day of the program (the first program is set for April 28) can participate afterward by accessing the program recording. To gain access to the recording(s), individuals need to register for the webinar.
