CHA Podcast Introduces Framework for Population Health Management
Master PHM one week at a time with CHA podcasts, briefs

Continuing CHA’s series on population health management, this week’s podcast and Snapshot introduce a seven-level framework for population health, providing detailed guidance on developing a financially sustainable role in population health management (PHM) through critical interrelated analyses and decisions.

During Level One, hospital and health system leadership teams gain important context and a clear understanding of what population health management is, as well as the care management imperative that will drive the coordination and management of patient care across the continuum. Teams also assess the roles providers can play, as described in last week’s Snapshot, and identify which should be explored further based on the organization’s strengths and weaknesses.
