CHA Medication Safety Committee Releases Anticoagulant Guideline
Summary of safe-use practices

CHA’s Medication Safety Committee has created a High Alert Medication Guideline summarizing safe-use practices for the anticoagulants unfractionated heparin, warfarin and enoxaparin. Anticoagulation is high-risk therapy that involves complex dosing and stringent monitoring, as well as ensuring patient adherence in the outpatient setting. The new document is intended only as a guideline. Hospitals should consult with their pharmacy and therapeutics committees prior to adopting the guideline’s recommended practices. The Medication Safety Committee is comprised of representatives from the California Department of Public Health, California Society of Health System Pharmacists, California Board of Pharmacy, related professional organizations and CHA member hospitals.
