CDPH Issues Discharge Planning Requirements

The California Department of Public Health has issued the attached All Facilities Letter, which notifies hospitals of requirements implemented by SB 675 (Statutes of 2015, Chapter 494), such as ensuring that discharge planning is appropriate based on the needs of the patient and other requirements related to patient-designated caregivers. The new law adds to existing hospital discharge planning policy requirements, including requiring hospitals to provide each admitted inpatient with the opportunity to identify a family caregiver and to notify that person of the patient’s discharge or transfer as soon as possible. The requirements become effective Jan. 1, 2016.
