CACHI Proposals Due April 29

The California Health and Human Services Agency and Community Partners are soliciting proposals for up to six accountable communities for health (ACHs) under the Accountable Communities for Health Initiative (CACHI). During its three-year span, CACHI will assess the feasibility, effectiveness and potential value of a more expansive, connected and prevention-oriented health system. Grant proposals are being accepted for the initiative’s first year, and should identify a set of short- and medium-term goals related to a specific health issue, such as a chronic condition like diabetes, cardiovascular disease or asthma.
